The position will engage patients at the medical visit, determine the extent of their needs for therapy and/or community assistance and/or referral to psychiatry. The patient will also engage patients of the Medication Assisted Recovery clinic to assess and determine the extent of their needs.

1. Master Degree in Social Work, Psychology or Counseling at time of hire
2. Licensed or License Eligible to practice Social Work or Professional Counseling in the State of Illinois
3. Sensitive to the needs of underrepresented minority populations.

• Preference will be given to applicants who have experience in a family medicine residency training program and/or federally qualified health center.
