This position will provide assistance to improve overall health and well-being to individual and populations of patients. This position will coordinate and manage patient care in conjunction with Federally Qualified Healthcare Center (FQHC) and Patient Centered Medical Home (PCMH) guidelines. This position will coordinate patient education. This position will collaborate with patients, families, the patient’s Primary Care providers and other members of the healthcare team to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services.

Salary: $4,738.78/month

1)  Licensed as a registered professional nurse in the State of Illinois; AND

2)  Bachelor’s degree; AND

3)  2 years of public health or specialized nursing experience

  • Licensed as a Registered Nurse by the Illinois Department of Registration and Education.
  • Understnading of Care Coordination role in a primary care setting.
  • Preference given to knoledge of primary care environmnet and local community health and social service resources.
